Residential Habilitation Services
"Group Home"

At The Family Tree Concept, Inc., we provide 24/7 Residential Habilitation Services in licensed group homes across Miami-Dade County for adults with developmental disabilities who require structured living, supervision, and skill-building supports. Services are delivered in accordance with the Florida Medicaid Waiver Handbook, Chapter 65G-5, and person-centered planning standards.

Each home is staffed by trained professionals and overseen by interdisciplinary teams, ensuring a safe, nurturing environment that fosters independence and clinical progress.

 

Standard Residential Habilitation

This level supports individuals who do not require behavioral interventions but benefit from consistent supervision and assistance with daily living.

Includes:

  • Training in Activities of Daily Living (ADLs), such as bathing, dressing, eating, and grooming

  • Assistance with medication administration per 65G-7

  • Household skill-building (laundry, chores, cooking)

  • Community integration and socialization activities

  • Daily routines that promote personal growth, health, and safety

  • Participation in the person-centered support plan

Authorized for individuals who require supervision and support but not behavior-focused services.

 

Behavior-Focused Residential Habilitation

This level is designed for individuals with mild to moderate maladaptive behaviors who benefit from structured behavioral programming and documentation.

Includes:

  • All services provided under Standard Residential Habilitation

  • Implementation of a Positive Behavior Support Plan (PBSP) developed by a certified Behavior Analyst

  • Daily behavior documentation and data tracking

  • Reinforcement strategies and redirection techniques

  • Staff trained in behavioral intervention and crisis prevention

  • Ongoing communication with the clinical team for fidelity and effectiveness

Authorized for individuals who exhibit behavior that interferes with daily functioning.

 

Intensive Behavioral Residential Habilitation

This is the highest level of care, reserved for individuals with severe or dangerous behavior that may include aggression, self-injury, property destruction, or crisis-level psychiatric involvement.

Includes:

  • Enhanced staffing ratios (1:1 or higher as approved)

  • 24/7 monitoring and on-site supervision

  • Direct implementation of LRC-approved PBSPs with restrictive or reactive strategies (when applicable)

  • High-frequency clinical oversight by a BCBA or Behavior Analyst

  • Coordination with psychiatric providers and medical professionals

  • Crisis response planning and documentation per Medicaid and APD standards

Authorized for individuals whose behavior presents a risk to themselves or others and who require intensive structure and behavioral oversight.

Staff Credentials & Oversight
  • All homes are staffed with Direct Support Professionals (DSPs) trained in CPR, First Aid, HIV/AIDS, Zero Tolerance, 65G-7 Medication Administration, and behavioral intervention techniques

  • Clinical and operational oversight is provided by the FTC management team, BCBAs, and licensed healthcare providers

  • All services are documented and reviewed for compliance with Qlarant, APD, and Medicaid standards

Behavior Analysis Services
(Levels 1, 2 & 3)

The Family Tree Concept, Inc. provides clinically driven, Medicaid Waiver-approved Behavior Analysis Services for individuals with developmental disabilities who exhibit maladaptive, challenging, or dangerous behaviors. Services are designed to improve functional outcomes and reduce behaviors that interfere with learning, safety, and community living.

Behavior Analysis Services are rendered by credentialed professionals under Florida’s Medicaid Waiver and are categorized into three levels based on complexity, frequency of behavior, and clinical intensity:

Level I – Assessment & Plan Development

This level includes initial evaluations and the development of a Positive Behavior Support Plan (PBSP) in compliance with APD and Medicaid standards.

  • Functional Behavior Assessment (FBA) conducted by a BCBA

  • Identification of behavioral excesses and skill deficits

  • Development of a BASP that includes proactive strategies, skill-building, and reactive procedures

  • Coordination with interdisciplinary teams, legal guardians, and caregivers

  • Initial staff training on BASP implementation

  • Submission of plans for Local Review Committee (LRC) approval when applicable

Authorized for individuals requiring new or revised behavior plans.

Level II – Ongoing Monitoring & Clinical Support

Level II services focus on regular monitoring and clinical oversight of an existing PBSP. These services ensure the behavior plan is implemented correctly and that data is reviewed for progress or necessary revisions.

  • Scheduled site visits and observations (home, ADT, school, or work)

  • Review of behavior data and progress tracking

  • Ongoing collaboration with family members and support staff

  • Minor revisions to PBSP as needed (not requiring full reassessment)

  • Training and fidelity checks with caregivers or direct service providers

Authorized for individuals with moderate behavioral needs requiring structured support and maintenance.

 

Level III – Intensive Behavioral Services

Level III is designed for individuals with severe or dangerous behavior, such as aggression, self-injury, property destruction, or behaviors that place themselves or others at immediate risk.

  • High-frequency behavior monitoring and intervention

  • Crisis plan development, including Reactive Strategies and Restrictive Procedures (when necessary and approved)

  • Comprehensive PBSPs with trauma-informed and evidence-based interventions

  • Frequent clinical visits and hands-on coaching for staff

  • Coordination with psychiatrists, mental health providers, and LRC when applicable

  • Emergency response and plan modifications following critical incidents

Authorized for individuals requiring intensive intervention and high-level behavioral oversight.

 

Oversight & Credentials

All behavior services at FTC are delivered by qualified professionals, including:

  • Board Certified Behavior Analysts (BCBAs)

  • Board Certified Assistant Behavior Analysts (BCaBAs) under supervision

  • In alignment with Chapter 65G-4, Florida Medicaid Waiver Handbook, and APD behavior services requirements

Behavior Assistant Services

Behavior Assistant Services at The Family Tree Concept, Inc. are provided under the supervision of a certified Behavior Analyst and are designed to support the implementation of approved Positive Behavior Support Plans (PBSPs). These services are essential for helping individuals with developmental disabilities learn replacement behaviors, increase adaptive skills, and reduce maladaptive behaviors in real-time environments.

All Behavior Assistant Services are authorized by the Florida Medicaid Waiver Program and delivered in compliance with Chapter 65G-4, the Medicaid Waiver Services Coverage and Limitations Handbook, and individual support plans.

 

Scope of Services

Behavior Assistants (BAs) provide direct, hands-on behavioral support in the home, community, or program setting (including group homes and ADTs). Their role is to implement the individualized behavior plan with fidelity and provide feedback to the supervising Behavior Analyst.

Key responsibilities include:

  • Direct implementation of the individual’s BASP

  • Continuous data collection on target behaviors and replacement skill performance

  • Use of reinforcement systems, prompting strategies, and de-escalation techniques

  • Redirection of maladaptive behavior using approved, non-restrictive interventions

  • Execution of reactive strategies (when authorized through LRC) under the behavior analyst’s guidance

  • Participation in staff training refreshers, fidelity checks, and team meetings

  • Contribution to monthly behavior reports and documentation

Qualifications & Supervision
  • All BAs are proficiency tested and trained according to APD standards

  • They must work under the supervision of a Board Certified Behavior Analyst (BCBA) or Board Certified Assistant Behavior Analyst (BCaBA)

  • Services are provided only when a valid PBSP is in place

  • Frequency and duration are authorized based on medical necessity, behavior severity, and LRC (if applicable)

 

Medicaid Waiver Compliance
  • Behavior Assistant Services are not therapeutic counseling, but behavioral support based on ABA principles

  • They are reimbursed based on face-to-face service delivery and must be documented daily

  • BAs do not modify plans but must implement strategies as written in the PBSP

  • Services must align with the consumer’s approved support plan, FBA results, and clinical recommendations

 

When Are Behavior Assistant Services Appropriate?
  • When a consumer has an approved BASP

  • When a consumer engages in frequent, disruptive, or dangerous behavior that interferes with quality of life

  • When in-the-moment support is needed to reinforce new skills or respond to behavior safely

  • When staff need real-time modeling of behavioral strategies and coaching from clinical teams

Life Skills Development I (Companion Services)

Life Skills Development I – Companion is a Florida Medicaid Waiver-approved service designed to support adults with developmental disabilities in achieving greater independence, community access, and personal growth. Companion Services are non-medical, supervision-based supports provided in the individual’s home or community, with a focus on social engagement, safety, and life enrichment.

At The Family Tree Concept, Inc., we offer Companion Services as part of our person-centered care model, helping individuals participate in meaningful activities and interactions that promote independence, inclusion, and dignity.

Purpose & Scope of Services

Life Skills Development I – Companion services are provided to individuals who do not require direct physical assistance with ADLs, but who benefit from guidance, redirection, monitoring, and social support during daily and community-based activities.

Companion Services may include:

  • Supervision and engagement during community outings (e.g., shopping, parks, recreational events)

  • Support in managing basic routines and schedules

  • Promoting safety awareness in home and community environments

  • Modeling appropriate behavior and communication skills

  • Assisting with participation in group activities or hobby-based tasks

  • Supporting social interactions and reducing isolation

  • Helping with non-complex tasks like organizing belongings, reading, or writing

 
Medicaid Waiver Requirements
  • Services must be delivered face-to-face, one-on-one

  • Individuals must not require hands-on personal care assistance

  • Activities must align with the person’s support plan and life goals

  • Companion staff must document daily service notes including date, time, activities, and outcome

  • May be provided in the home or integrated settings in the community

  • Supervision must be related to functional or safety needs as determined by APD

 

Note: Companion Services cannot include medication administration, bathing, feeding, lifting, or transportation unless authorized separately.

FTC Companion Services are delivered by trained Life Skills Trainers who are certified in:

  • APD Core Competencies

  • Zero Tolerance, HIPAA, and Client Rights

  • CPR and First Aid

  • Community inclusion and safety awareness

 

All staff receive additional agency training to reflect FTC’s commitment to relationship-based care and therapeutic environments.

 
When to Consider Companion Services

 

This service is ideal for individuals who:

  • Require monitoring for safety or behavior, but not physical care

  • Benefit from social interaction and structured routines

  • Need encouragement and support to access their community

  • Are at risk of isolation or disengagement

  • Thrive in one-on-one support environments with positive role models

Life Skills Training (Adult Day Training - ADT)

Life Skills Development III – Adult Day Training (ADT) is a Medicaid Waiver-authorized service that offers structured, group-based programming for adults with developmental disabilities. The goal of ADT is to support each individual in acquiring, maintaining, and enhancing independent living skills, community integration, social development, and functional abilities that align with their personal goals and support plan.

At The Family Tree Concept, Inc., our ADT program provides a safe, therapeutic, and engaging environment where participants are not just supervised — they are empowered, engaged, and respected.

Program Focus & Activities

Our ADT curriculum is built to promote:

  • Communication and socialization

  • Personal hygiene and self-care skills

  • Domestic living and daily routines

  • Health and safety awareness

  • Pre-vocational and community readiness skills

  • Recreation, creative arts, and physical movement

  • Self-advocacy and emotional regulation

 

Each participant receives a daily schedule tailored to their interests and developmental level. Group instruction is paired with individualized support to promote real-world skill generalization.

Medicaid Waiver Requirements

ADT services at FTC are provided in accordance with the Florida Medicaid Waiver Handbook and Chapter 65G rules:

  • Group ratios are maintained based on waiver authorizations and individual needs

  • Services are provided during daytime hours, typically Monday through Friday

  • Each participant must have an Individual Implementation Plan (IIP) aligned with their Support Plan

  • Daily attendance and progress notes are documented, reviewed, and maintained

  • Clinical integration is available when behavioral or mental health needs are identified

  • Transportation to and from ADT may be authorized separately if needed

 

Note: ADT is a non-residential service. Services may not include hands-on personal care or therapy unless authorized through other Medicaid Waiver services.

Clinical & Behavioral Integration

As a clinically integrated agency, FTC offers:

  • In-program behavioral support by BCBAs and Behavior Assistants

  • Coordination with mental health professionals and guardians

  • Training for ADT staff on BASP implementation, safety protocols, and data tracking

  • Seamless communication with residential staff and WSCs for continuity of care
